📝 Important: Please Read Before Requesting Dates

This form is a request, not a confirmed appointment. Please do not begin trapping until our team has reviewed your request and sent you an official confirmation email.

💡 What if your trapping schedule changes? We know trapping can be unpredictable. If you have an active request on file and suddenly wake up to a full trap without a confirmed date yet, don't wait — bring the cat directly to our Philadelphia location during our standard check-in hours (**Monday–Saturday from 7:30 AM to 8:30 AM**) and we will do our best to accommodate you.

🚫 Important Notice regarding unexpected catches: Please note that this location operates on a strict, pre-scheduled appointment basis and **cannot accept walk-ins**. Because surgery days vary and space is limited, we cannot guarantee intake or medical availability without a confirmed reservation. Please wait for our team to contact you before arriving.

Desired Appointment Date(Required)

Please select a target date at least 3 business days in advance to allow our team time to process and confirm your request.

How many cats?(Required)

Please enter a number from 0 to 5.
